Your thoughts matter!  After your Individual Program Plan (IPP) meeting or when you receive your completed (IPP), you and your family will see a link to a short survey at the end of the document.

The survey is important because it helps the Department and regional centers learn what is working well and what needs to improve.  The IPP is your plan for services and supports, and your feedback helps make the next IPP experience better for you and other people.

Why take the survey? 

  • It only takes a few minutes.
  • Your answers are private.
  • It gives you a chance to share what worked and what could be better, so that improvements can be made.

How your feedback helps: 

Every survey response makes a difference.  The more families share their experiences, the better the Department and regional centers can understand what supports are most helpful and where changes are needed.

What to expect: 

  • The survey link is found at the end of the IPP.
  • You can do the short survey online or request a paper version.
  • Families and individuals are encouraged to help make IPPs better!
