The Quality Incentive Program (QIP) data collection has begun. Help us make sure your service provider doesn’t miss out on this opportunity to earn their full rate for what they do for you! You can share the information below with them.
As of November 3, 2025, electronic surveys have been sent by email to service providers who served individuals between July 1, 2024, and June 30, 2-25 and delivered a service included in rate reform.
The Incentive: While survey and QIP participation is voluntary, service providers who complete and submit their surveys by the January 31 deadline will earn the 10% quality incentive rate component of their applicable rate model in FY 2026-27.
About the Surveys
More than 18,000 surveys have been sent, with more continuing to be sent throughout November. All eligible providers received a Provider Capacity survey, which gathers information on staffing, organizational structure, available languages for services, and other related topics.
In addition to the capacity survey, some providers received questionnaires based on their type of services:
- Employment providers received a separate employment survey focusing on job support for individuals served and staff training.
- Residential providers received a separate prevention and wellness survey, gathering information on individuals’ preventative health screenings and immunizations.

The Department’s Office of Quality Assurance will schedule public trainings and office hours to answer questions during the data collection period between November 2025 and January 2026. Registration information will be posted on the stakeholder events webpage.
